Call
InsertPage(insertPosition)
method to add a new page in desired position (http://www.colorpilot.com/pdfcreatorpilotmanual/I PDFDocument4/InsertPage.html).

Here is an example (VB Script) how to open a document and insert new page:

Set PDF = CreateObject("PDFCreatorPilot.PDFDocument4")
PDF.Open "test.pdf", ""
PDF.InsertPage 1 ' zero-based index (0 - 1st page, 1 - 2nd...)
PDF.CurrentPage = 1
PDF.ShowTextAt 20, 20, "Here is a new page."
PDF.SaveToFile "test.pdf", true
Set PDF = Nothing
