Two Pilots forum / Digital photo software / Import fails in Exif

I'm using Excel as the import. I've tried with and without the leading / for each line. Either way says it works, yet it imports nothing. These are fields that are not presently in the file.

Also, which is the format for the excel file that I should use? With or without the leading / ?

/Exif/Photo/ISOSpeedRatings 640
/Exif/Photo/ExifVersion 0221

or

Exif/Photo/ISOSpeedRatings 640
Exif/Photo/ExifVersion 0221

Can you maybe even attach a sample import excel file and also a sample import csv file? I learn well from example. :)
